Situated near downtown Huntsville and the medical district, our office is four streets east of Huntsville High School and right off Bob Wallace Ave. If you have any trouble finding us, please refer to the Google Maps link at the bottom of any page on this website!

To keep wait time to a minimum, we request patients arrive 15 minutes prior to their scheduled appointment time. This allows for new and existing patients to complete medical history and insurance updates, when needed.

To help us keep your patient file up to date, please be sure to bring your photo ID and insurance card(s) when you visit our office. While Dr. Perkins is not an in-network provider, we will always file insurance claims on your behalf to ensure eligible treatments are covered.

* We cannot guarantee what each insurance company/plan will cover. It is each patient’s responsibility to understand the terms and coverage of their policy.

Though we do not prioritize walk-ins over standing appointments, our team will always do our best to accommodate urgent needs. If you have a dental emergency please give us a call during business hours at (256) 534-3560 and we will work to schedule you a timely appointment.
